Forms for 2023-24 2023-24 IRS 4506T FORM- REQUEST TAX TRANSCRIPT (PDF, 175 K) Student/Parent must mail to IRS. See page 2 for address. Transcript will be mailed to the home address. Submit to our office the transcript(s). 2023-24 STUDENT NON-FILING FORM (PDF, 175 K) Student must request from IRS a letter stating that she/he did not file a 2021 Federal Tax Return. Student must mail the 4506 T to IRS. See page 2 of where to mail this form. 2023-24 VERIFICATION WORKSHEET 4 (PDF, 296 K) This form must be notarized. Submit a copy of high school diploma and/or GED along with copy valid government ID. 2023-24 AUTHORIZATION TO RELEASE INFORMATION (PDF, 175 K) 2023-24 DATA FORM (PDF, 267 K) 2023-24 DEGREE OBJECTIVE CHANGE FORM (PDF, 215 K) 2023-24 FINANCIAL AID APPEAL FORM (PDF, 216 K) Must be signed by Advisor. Submit all documentation under Directions. Incomplete appeals will not be submitted to the financial aid appeals committee. 2023-24 FRESH START INITIATIVE RELEASE FORM (PDF, 169 K) 2023-24 INCOME/EXPENSE BUDGET FORM (PDF, 344 K) 2023-24 PARENT NON-FILING FORM (PDF, 175 K) Parent(s)must request from IRS a letter stating that she/he did not file a 2021 Federal Tax Return. Parent(s) must mail the 4506 T to IRS. See page 2 of where to mail this form. 2023-24 PROOF OF IDENTITY / EDUCATIONAL PURPOSE FORM (PDF, 236 K) This form must be notarized and mailed to our office along with a copy valid government ID. 2023-24 SPOUSE NON-FILING FORM (PDF, 173 K) 2023-24 UNUSUAL ENROLLMENT (PDF, 342 K) Official transcript from each college/university required. 2023-24 VERIFICATION WORKSHEET 1 (PDF, 270 K) Submit a copy of 2021 Federal Tax Returns and 2021 W-2s. If applicable spouse's and/or parents tax information 2023-24 VERIFICATION WORKSHEET 5 (PDF, 374 K) Must be notarized. A copy of 2021 Federal Tax Returns and W-2s for spouse and/or parents if applicable. A copy of high school diploma and/or GED along with valid ID must accompany the form. GBC CONSORTIUM AGREEMENT (PDF, 375 K) This form is only if Pell Grant eligible.

Forms for 2024-25 2024-25 DATA FORM (PDF, 268 K) 2024-25 FINANCIAL AID APPEAL FORM (PDF, 242 K) Advisor must sign off on form. Submit GBC unofficial academic transcript, What-If report, and personal essay with the form. Incomplete appeals will not be submitted to the financial aid appeals committee. 2024-25 INCOME/EXPENSE BUDGET FORM (PDF, 394 K) The 1st two pages has the instruction on how to complete this form. 2024-25 PROOF OF IDENTITY / EDUCATIONAL PURPOSE FORM (PDF, 235 K) This form must be Notarized and mailed to GBC Financial Aid Office along with a copy of valid driver's license. 2024-25 VERFICATION WORKSHEET 1 (PDF, 272 K)
